To nominate a family for Olivia's Big Splash Custom Bath Giveaway please complete and submit the entry form below. Families with a child with special needs may nominate themselves by providing a child services provider as a reference. Nominations may also be made by a professional in the field of children’s services. The nominee must reside in one of the following counties: Jackson, Clay, Cass, or Platte in Missouri, and Wyandotte or Johnson in Kansas.

Your nomination package must include the nomination form with one photo of the child (jpg), two photos of the bathroom to be remodeled (jpg).
